Candidate record forms
These are for:
- students and teachers to authenticate the work
- teachers to record their assessments and supporting information.
Forms are published approximately two years before the year of assessment. These forms can also be accessed through the assessment resource section of the relevant subject page.

Centre Declarations
Centre declaration sheet (CDS)
This is to confirm:
- authenticity of students' work
- that internal standardisation of marking has been carried out.
These requirements do not apply to GCSE English Language Spoken language endorsement (8700/C) or A-level biology, chemistry and physics practical skills endorsements (7402/C 7405/C 7408/C)which instead required a NEA, fieldwork and live performance centre declaration form.
OnlineCDS pop-up in Centre marks submission.
When you click the ‘Submit to AQA’ button to finalise your mark/grade submission, a pop-up will appear onscreen asking you to confirm on behalf of the teacher responsible for the assessment that:
- your marks/grades have been given in accordance with the requirements of the specification
- your centre has taken every reasonable step to ensure that the work presented is that of the named candidates
- any assistance given to candidates beyond that given to the class as a whole and beyond that described in the specification has been recorded on the Candidate Record Form and has been taken into account. The marks/grades given accurately reflect the unaided achievement of the candidates
- your centre has standardised assessment across different teachers/assessors and teaching groups to ensure that all candidates have been judged against the same standard. If two or more teachers/assessors are involved in marking, one of them must be designated as responsible for standardising the assessments of all teachers/assessors at the centre.
For Project qualifications, there will be an additional statement to confirm that all candidates entered have completed entirely new work or have substantially reworked a project for which they have already received a result in a previous exam series.
Physical CDS form.
In additionto the online declaration, schools must complete a physical Centre Declaration Sheetand retain it within their school. For GCSE and A-level PE only, a copy of the Declaration Sheet should be given to their allocated moderator.
NEA, fieldwork and live performance centre declaration form
This is used to confirm:
- work has been checked to ensure it is age appropriate and does not raise any safeguarding concerns
- that various specification requirements have been met.
Schools entering candidates for a GCSE, AS level, A-level or Project qualification that includes NEA or an Endorsement must complete an NEA, fieldwork and live performance centre declaration form.
This form must be signed by your head of centre and emailed to NEAdeclaration@aqa.org.uk
The declaration form has additional subject specific statements for:

Performance duration declarations – Dance, Drama and Music
Theseare used to confirm that candidates have met the minimum performance times as required by the subject and to notify us of any candidates that don’t.

Practical programming statement – GCSE Computer Science
This is used to confirm that candidates have had an opportunity to undertake a programming task or tasks during their course of study.
